Requirements:

  1. Birth Certificate
  2. School Results
  3. KTP of both parents (for Indonesians)
  4. Passports and Kitas of parents and children (if foreigners)
  5. Photos of child/ren 4x6 (3 pcs) and 2x3 (3 pcs)
